As @"John Hudson" said, it goes back to the typewriter. But the link between typewriters and modern computers is the teletype keyboard, which led to typewriter quotes becoming part of ASCII.2 -

Technically, a TTF font can have the .otf extension if it has GSUB/GPOS tables. I can't remember why at the moment, but few do in practice.2 -

Perhaps because it was at one time so popular in the U.S. If you look at printed media from the decades around 1900, it was everywhere. So, it becomes "american" by association.3 -
